SCOPE
Seamless Care Optimizing the Patient Experience (SCOPE) is a virtual interprofessional health team that supports primary care providers through a single point of access. 

Registered SCOPE providers will connect with a dedicated telephone service to access support from internists, diagnostic imaging, NYGH resources and community services.

Online Appointment Booking (OAB)

NYTHP is applying for additional funds to support onboarding and funding of OAB. Currently, there are two separate funding programs: NYTHP for Ocean or Medeo, and OSCAR Ontario for Cortico or Veribook. This is an opportunity for clinics that do not currently use OAB to receive funding and support for OAB implementation and licensing costs until March 31, 2023.

Remote Patient Monitoring (RPM)

North York General Hospital (NYGH) has established a Remote Patient Monitoring (RPM) virtual care program, which aims to protect patients and prevent hospital readmissions for patients positive with COVID–19.

Family physicians can refer and/or positive COVID-19 patients can self-refer themselves into the program. This can occur by filling out a simple online referral form on the NYGH website. More information, including details on the RPM partnership with the North York Toronto Health Partners (NYTHP) and other virtual care applications, please visit the NYGH website.

Seniors



NY Cares
North York Toronto Health Partners (NYTHP) has launched a collaborative initiative to offer seamless, tailored care to help patients transition from hospital to home. North York Community Access to Resources Enabling Support (NY CARES)provides care and support for patients, who would otherwise be in hospital, so they can safely stay at home while waiting for a bed in long-term care or another setting.

RAAM Clinic
Rapid Access Addiction Medicine (RAAM) Clinic to help care for patients seeking treatment for any substance abuse disorder. RAAM clinics are drop-in clinics and do not require an appointment.

RAAM clinics offer quick access to quality care to help patients manage addictions.

Palliative Care



The Edwin S.H. Leong Geriatric Supportive Care Outreach Program

This program enables adults living with advanced, non-cancer life-limiting illness, whose wish is to stay at home for as long as possible or to die at home to receive in-home care from an outreach physician with the support of a nurse practitioner and other community supports as needed.

We provide:

  • Seamless care, 24 hours a day and 7 days a week
  • Service within the following catchment area: south of Steeles Ave., north of Lawrence Ave., west of Victoria Park Ave., and east of Bathurst St. If you live outside of the catchment area, please speak with your family doctor or home care coordinator for local palliative care support.
