NY Cares
North York Toronto Health Partners (NYTHP) has launched a collaborative initiative to offer seamless, tailored care to help patients transition from hospital to home. North York Community Access to Resources Enabling Support (NY CARES)provides care and support for patients, who would otherwise be in hospital, so they can safely stay at home while waiting for a bed in long-term care or another setting.

Palliative Care



The Edwin S.H. Leong Geriatric Supportive Care Outreach Program

This program enables adults living with advanced, non-cancer life-limiting illness, whose wish is to stay at home for as long as possible or to die at home to receive in-home care from an outreach physician with the support of a nurse practitioner and other community supports as needed.

We provide:

  • Seamless care, 24 hours a day and 7 days a week
  • Service within the following catchment area: south of Steeles Ave., north of Lawrence Ave., west of Victoria Park Ave., and east of Bathurst St. If you live outside of the catchment area, please speak with your family doctor or home care coordinator for local palliative care support.

Remote Patient Monitoring (RPM)

North York General Hospital (NYGH) has established a Remote Patient Monitoring (RPM) virtual care program, which aims to protect patients and prevent hospital readmissions for programs such as surgical transitions and North York at Home (home care program).

More information, including details on the RPM partnership with the North York Toronto Health Partners (NYTHP) and other virtual care applications, please visit the NYGH website.
