Remote Patient Monitoring (RPM) North York General Hospital (NYGH) has established a Remote Patient Monitoring (RPM) virtual care program, which aims to protect patients and prevent hospital readmissions for patients positive with COVID–19. Family physicians can refer and/or positive COVID-19 patients can self-refer themselves into the program. This can occur by filling out a simple online referral form on the NYGH website. More information, including details on the RPM partnership with the North York Toronto Health Partners (NYTHP) and other virtual care applications, please visit the NYGH website. NY Cares
North York Toronto Health Partners (NYTHP) has launched a collaborative initiative to offer seamless, tailored care to help patients transition from hospital to home. North York Community Access to Resources Enabling Support (NY CARES)provides care and support for patients, who would otherwise be in hospital, so they can safely stay at home while waiting for a bed in long-term care or another setting. |
