PCN Membership Agreement 2023
The goal of the NYTHP Primary Care Network is to provide a forum for communication, dissemination of information and to represent the interests and the voice of the primary care community within North York.
Our vision: Advancing excellence in health care for everyone in North York. Co-creating a system of care that is team-based, integrated, accessible and equitable.
Our mission: Transforming our health system by bringing primary care together to shape the design and delivery of programs and services. Through our shared leadership, collective voices and community partnerships, we strive to promote better health outcomes and positive experiences for all.
By joining the NYTHP PCN I understand the following membership benefits and rights:
PCN membership is voluntary for any primary care providers with patients who use services in the NYTHP OHT region;
Membership in the NYTHP PCN will not be legally binding and primary care providers may withdraw at any time;
PCN members shall elect the Primary Care Advisory Council (PCAC) to represent the broader PCN membership;
Members shall receive communication updates regarding relevant initiatives and programs in the NYTHP OHT.

Benefits of Joining:
Q: Do I have to join the Primary Care Network?
A: Joining the PCN is completely voluntary and a great opportunity for primary care clinicians in our community to come together as a group to help shape an integrated health care system.
Q: Is there a cost to joining the Primary Care Network?
A: No! There is no cost to joining the PCN.
Q: What is the risk to joining a Primary Care Network?
A: There is no risk to signing on to a physician network. Withdrawal from the PCN can occur at any time with 30 days notice.
Q: How will my voice as a community physician be heard?
Q: Will membership in the PCN affect my remuneration or enrolment model?
A: No – the OHT does not have jurisdiction over physician compensation, patient enrolment models or rosters.
