Our Purpose

  • To provide a united voice for Primary Care at the OHT governance and decisions making tables as we co-design a more integrated health care system
  • Allow members to elect representatives to the Primary Care Advisory Council to share their ideas and concerns.
  • To provide a community of practice for sharing of resources, best practices and data
  • Provide opportunities for leadership as we navigate new care models
  • Establish a two-way communication structure between community care providers and those at the decision-making tables
  • Allow physicians to lead and self-govern as we know best the journey our patients take through the health care system
